Before we can deliver equipment to you, we do need to know just a little bit more about you. If you’re a new client, please take a moment to download both the Credit Card Authorization form and the Credit Application. Fill those out and fax them back to us at 310-979-3503.
Also, because our equipment is so very valuable, we’d like to know that your insurance is sufficient to cover accidental loss. Please ask your insurance company to provide an insurance statement, similar to the sample insurance form, below.
